1. I give permission for Citipointe Church to obtain emergency medical, hospital or ambulance service at any time they consider necessary. I acknowledge that I will be liable for any medical, hospital or ambulance expenses incurred in the treatment for my child. Citipointe is released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to my child.
2. Attendees are not permitted to leave the event venue. Citipointe Church, its employees, and volunteers as organisers of Citipointe Church, take no responsibility for your child choosing to leave the event venue independently.
3. Transport: I agree to allow my child under the supervision of Youth Society Redcliffe to drive with a Grace Lutheran College bus driver who is licenced for the vehicle in which they will be travelling.
